A35-year-oldwomanwithchronicmedialkneejointpainwasreferredformagneticresonancearthrography(MRA).

RadiologicFindings：

A"grabbed"fluoroscopicimage(Fig.77A)showsaconvexscleroticrimcephaladtoandovoidradiodensity,whichissurroundedbyaradiolucentlineprojectedthroughthelateralaspectofthemedialfemoralcondyle.T1fat-saturatedimgesafterintra-articularinjectionofdilutegadolinium(Figs.77B,coronal；77C,transaxial；and77D,sagittal)showadiscretefragmentedlesionofboneandcartilageintheposterolateralaspectofthemedialcondyle.Gadoliniumisseentounderminethefragmentover~75%ofitssurfacearea,especiallyposteriorly.Asubtlelinearlow-signalalterationisevidentwithinthecartilageatthemedialmarginofthelesion,whereasthecartilageisdiscontinuouslaterally(Fig.77B).Thesagittalimagerevealsosseousdiscontinuityanteriorlyandposteriorly.

Diagnosis：

Osteochondritisdissecans(OCD).

DifferentialDiagnosis：

Spontaneousosteonecrosis.
